*If for any reason your booked appointment day/time can not be honored, (with no reschedule options that accommodate both parties) by Jessica and/or House of Self, we will reach out via email and refund in full any deposits/fees paid. **While we always strive to provide a great experience for all our clients, we reserve the right to refuse service if necessary to ensure the safety and comfort of everyone.
Service Agreement
House of Self and Client Agreement and Informed Consent Disclosure Statement for Services
This agreement is intended to provide important information to you regarding House of Self energy healing services. Please read the entire document carefully and be sure to ask your practitioner any questions you might have regarding its contents.
Information about your practitioner Jessica Goldfield:
Jessica has been trained in Usui Ryoho Reiki by reiki master/teacher Lisa Powers completing her master/teachers certification as Shinpiden Reiki Master & Teacher on April 28, 2019. You can also find her registered with the International Association of Reiki Professionals. Jessica also has a certification as a Crystal Practitioner under the Hibiscus Moon Crystal Academy awarded on December 29, 2021.

Liability
This agreement is for Energy Healing, not any other form of mental health care practices such as, but not limited to, Counseling, Psychology or Psychotherapy. Jessica Goldfield is working only within the capacities of a Reiki Master and will not be held liable for discrepancies.
Cost of Services
House of Self services are not covered by insurance. Payments are due at the time of booking your appointment. All sessions conducted on Zoom (Video or Voice call-whichever you prefer), except for the in person reiki session.
Confidentiality
As a practitioner, it is my duty to protect the confidentiality of the communications with my clients, but it must be understood that there is limited confidentiality when sessions are conducted online, as I can not guarantee the secure connection of the internet and sites used.
Scheduled Session Appointment Cancellation Policies
A 24-hour advance notice is required if you must cancel or reschedule your session’s appointment.
****In any case, if a session was missed without 24 hours prior notice due to an emergency, please contact me soon as you are able so we can discuss a financial grace.. I understand life happens and in an emergency, I don’t want you to stress over a missed session. I ask though that you please let me know at your earliest convenience.****
Our policies are created to allow for effective scheduling and to ensure all clients wishing to be seen may be accommodated. Please help me better serve you by notifying me as soon as possible if you must change or cancel your appointment.

About the reiki code of ethics
IARP Code of Ethics for Reiki Practitioners and Reiki Master Teachers
The Registered Reiki Practitioner (RP) / Registered Reiki Master Practitioner and Teacher (RMT) agrees to:
1. Abide by a vow of confidentiality. Any information that is discussed within the context of a Reiki session is confidential between the client and practitioner.
2. Provide a safe and comfortable area for sessions or classes and work to provide an empowering and supportive environment for clients and students.
3. Abide by all state/province/territory, national, local or county laws and rules, if any, regarding Reiki and any complementary integrative modalities that you practice.
4. Always treat clients and students with the utmost of respect and honor.
5. Have a pure and clear intention to offer your services for the highest healing good of the client and highest potential of the student.
6. In Reiki practice, provide a brief oral or written description of what happens during a session and what to expect before a client’s initial session.
7. In a Reiki course, provide a clear written description of subjects to be taught during each level of Reiki prior to class and list what the student will be able to do after taking the class.
8. Be respectful of all others’ Reiki views and paths.
9. Educate clients/students on the value of Reiki and explain that sessions do not guarantee a cure, nor are they ever a substitute for qualified medical or professional care. Reiki is one part of an integrative healing or wellness program, meant to enhance traditional treatment protocols.
10. Suggest a consultation or referral for clients to qualified licensed professionals (medical doctor, licensed therapist, etc.) when appropriate.
11. Never diagnose or prescribe. Never suggest that the client/student change prescribed treatment or interfere with the treatment of a licensed health care provider.
12. Be sensitive to the boundary needs of individual clients and students.
13. Never ask clients to disrobe (unless in the context of a licensed massage therapy session at the client’s option). Never touch the genital area or breasts. Practice hands-off healing of these areas if treatment is needed.
14. Be working to create harmony and friendly cooperation between Reiki Practitioners/ Master Teachers in the community and represent the IARP in a most professional manner.
15. Act as a beacon in your community by doing the best work possible.
16. Work to empower your students to heal themselves and to encourage and assist them in the development of their work with Reiki or their Reiki practices. Be available for questions and assistance after students complete a Reiki course.
17. Be actively working on your own healing so as to embody and fully express the essence of Reiki in everything that you do.
